Case #213 - Commission book from H.M.S. Undine

We are looking for a commission book from the Unidne dated 1959/1960, at which time the ship was serving in Cyprus.
The H.M.S. Unidne was a Royal Navy U-class destryoer which first saw active service during World War II.
Can anyone help?
